All nominees will be reviewed by the Budo Magazine Board of Trustees. Please supply adequate information. Applications must be submitted with a short bio, 100 words or less, and a color photo.
These can be attached to this form using the attachment fields near the end of this form. Make sure your photo is no larger than a 4x6 format if you are scanning it or no larger than 2mb if it is a digital photo. Click poster image at left to view larger.
